Please note that you can make feedback anonymously if you choose. Advocates/supports are available to assist you in making feedback if needed.
Feedback can be made to the NDIS Commission by:
- Phoning: 1800 035 544 (free call from landlines) or TTY 133 677. Interpreters can be arranged.
- National Relay Service and ask for 1800 035 544.
- Completing a feedback contact form.

Confidentiality Statement:
The information collected through this form is intended to gather feedback on our services. We take your privacy and data security seriously. All data collected will be treated confidentially and used solely for the purpose of improving our services. We use 'Google Forms' to manage our NDIS Feedback. Google Forms submissions are not public by default. Only the owner of the form (Integrity) can see responses unless you share them. The only information that we will see is data that is provided by you.
Participant Feedback Options:
- You have the option to complete this feedback form personally.
- If you prefer, you can choose to have a support person complete this form on your behalf.
- After providing feedback and before submitting you will have the option of providing your feedback anonymously.
